In this section you will find a variety of HR forms to support your HR operations from recruitment to performance/development and termination.

Recruitment request form

You may use the Recruitment Request Form to obtain relevant approvals to recruit for a position within your business. Often used to capture approvals by the Business Leader and Finance Lead to ensure the remuneration/benefits are aligned to budget and the role details are aligned to business strategies.

Reference check template

This Reference Check Template provides a guide on how to conduct a telephone reference check for prospective candidates, including telephone scripts and questions to help you ascertain the best fit for your business. It is recommended 2 reference checks are done during the recruitment process before an offer of employment.

Application for employment form

This Application for Employment Form is used to accompany a candidates application, ensuring any initial contact details/information are obtained, ensuring candidates have relevant working rights and to confirm the candidates availability. You may wish to add any screening questions to ensure candidates meet the minimum requirements for the role.

New employee details form

The New Employee Details Form is used to capture all relevant information required when engaging a new hire and needed to administer their pay or setup their employee record. Details may include; banking, tax and superannuation information, along with emergency contact details.

Position description template

This Position Description Template is used to capture an employees roles and responsibilities for their position, along with any qualifications, skills or experience that is required to carry out the role. This template also allows you to capture the Key Performance Indicators (KPIs) of the role to set clear expectations.

Interview questions

This Interview Questions guide allows you to set clear selection criterias to be used during interviews, allowing a fair and consistent recruitment approach for candidates. It also provides managers a guide on how to prepare and conduct interviews so that the best candidate may be picked for your business.

Phone screen guide

This Telephone Screening Guide may be used as part of your selection process for recruitment prior to conducting an interview. It enables you to determine candidate suitability, whether they have met the minimum role requirements and an opportunity to sense whether you would like to meet them for an interview.

Leave form

Used in conjuction with your Leave Policy, this Leave Form is used by an employee to request time off from work. This form is designed to capture the relevant leave type being requested, days and dates and allows for relevant approvals.

Request for parental leave

The Request for Parental Leave form is used by an employee to request parental leave where your employee is the primary care giver, and used in conjunction with your parental leave policy. This form is designed to capture all relevant dates, guidance surrounding notice and allows for relevant approvals.

HR Outsourcing

Request to cash out leave

The Request to Cash Out Leave form is used by an employee to submit a request to cash out annual leave as a lump sum payment and is designed to meet the record keeping requirements for cashing out annual leave. If your employee is covered under an Award or EBA, it is important to check this first before

Request to vary parental leave

The Parental leave – application to vary is used by an employee to change their original parental leave request relating to the initial 12 months, and used in conjunction with your parental leave policy. This form captures all relevant dates, provides guidance surrounding notice requirements and allows for relevant approvals.

Exit interview

The Exit Interview form is used during the termination process of an employee to collect feedback about the employee’s experience. This form is designed to ask questions that enable real and tangible examples to be obtained to identify opportunities to improve retention and engagement.

Expense form

Used in conjunction with your Expense Claim Policy, the Expense form is used as a means to facilitate reimbursements due to an employee for expenses incurred in the performance of their duties for your business, enabling the employee to submit a claim and ensuring record keeping requirements have been met.

Payroll deductions form

With the exception of deductions allowed under relevant laws (e.g. taxes, child support etc.), all other payroll deductions must have written permission from the employee prior to a deduction being made. This Payroll deduction form may be used by an employee to permit or request payroll deductions to be made.

Payslip

All businesses are required by law to supply their employees a Payslip when a payment for work has been made, with specific information required to be captured/shown on the Payslip. This form ensures all relevant information is included to meet your employer obligations.

Timesheet template

All award covered employees are required to record their start, finish and break times to ensure correct payment of salary or wages. This Timesheet template may be used to meet this record keeping requirement and may also be used for payroll payment calculations.

HR Services

Super salary sacrifice agreement

Employees may request to salary sacrifice a portion of their salary/wage before tax is deducted to their superannuation. To be able to do this, there are certain record keeping requirements that must be met. This form ensures these requirements are met and also enables employees to submit such requests.

MV accident report

During the course of employment, your employee may be involved in a motor vehicle (car) accident. In this event, the MV accident report form may be used to record the incident details. These details may also be need for insurance purposes.

Hazard report form

To meet your employer WHS/OHS obligations under relevant laws, all hazards identified need to be recorded, risks assessed and relevant controls taken to minimise or reduce any risks. This Hazard report form is designed to ensure all required steps are taken and recorded.

Risk management register

To meet your employer WHS/OHS obligations under relevant laws, and record keeping requirements, businesses must keep a register of all hazards and/or risks that pertain to your business operations. This Risk management register is designed to ensure this obligation is met.

Safe work procedure template

To meet your employer WHS/OHS obligations under relevant laws, operations/tasks that have a medium to high risk involved are required to have safe work procedures in place to clearly identify the safest way to conduct the task. This Safe work procedure template is designed to meet this obligation.

Return to work plan

Used in conjunction with your Return to Work/Rehabilitation Policy, this return to work plan template captures the plan for an employee to return to work after illness or injury. It allows for any work restrictions/job modifications to be documented and sets clear expectations for both employees and managers.

Incident report form

To meet your employer WHS/OHS obligations under relevant laws, all incidents and injuries which have occured need to be recorded/documented with relevant risk management to be undertaken thereafter. This Incident/Injury report form is designed to ensure all required steps are taken and recorded.

Individual learning plan

An individual learning plan is a working document used for employee development to capture the program or strategy of education or learning for the employee. This form is designed to include the planning, delivery and evaluation of an educational or learning program.

Monthly check in

As part of the performance review process, managers may choose to have monthly check ins to review the employee’s performance and gain valuable feedback. This form is designed to capture whether there any areas that require attention, such as additional training/resources or to highlight any concerns.

Probationary review

New employees who are due to complete a period of probationary employment (‘probation period’) are required to have a review to determine their suitability for continued employment. This Probationary review form enables managers to make this assessment and discussion record notes accordingly.

Career and development discussion

It is important that managers discuss development and career objectives with employees to understand the employee’s development needs. This form is a conversation guide for managers which is designed to ensure that managers have a firm understanding of each employees career aspirations.

Performance review – leader

Annual performance reviews/evaluations promote communication and provide useful feedback about job performance, to facilitate better working relationships, to provide an historical record of performance and to contribute to professional development. This form is designed to aid this process, ensuring Leaders are measured against core competencies of their role.

Annual performance reviews/evaluations promote communication and provide useful feedback about job performance, to facilitate better working relationships, to provide an historical record of performance and to contribute to professional development. This form is designed to aid this process, ensuring employees are measured against core competencies of their role.

Training and development request form

This training and development request form may be used by employees or managers seeking to submit a request to your business which may require employer support, such as financial funding. This form allows for the training/development request to be evaluated, training providers to be assessed and costs to be approved.

Training evaluation form

This training evaluation form allows for training courses/programs to be assessed to ensure it has provided the relevant training required, that this training was delivered successfully and to also obtain valuable feedback on how this may be improved or whether your employee found it useful.

Hours of work agreement/variation

This hours of work agreement/variation form may be used to gain agreement for employer-driven changes to work patterns, hours or days. It may also be used by an employee to request flexible working arrangements surrounding a change of work patterns, hours or days.

Request for flexible work arrangements

This form is used by an employee to request a change to their current work conditions. Requests may be made for carer or child care arrangements, for a disability, are 55 or older, are experiencing family or domestic violence or to provide such support to a family/household member.

Flexibility agreement

This flexibility agreement may be used by an employee with their manager to capture requests for flexible working arrangements that differ to the employee’s current work conditions. The form is designed to capture the reason and to allow the business to make appropriate considerations for the request.

Complaint and Grievance

Employee complaints and grievances should always be taken seriously and investigated accordingly. This form allows employees to report any instances of bullying/harrassment, discrimination or to highlight any grievance they may have. Information used on this form may then assist the employer in investigating the complaint/grievance.

Covid safe plan

This Covid safe plan template can be utilised to document the safety measures you have in place to limit the spread of Covid-19 at your workplace.

Risk Assessment

This form is used to conduct a risk assessment for a specific task or activity. It should be used in conjunction with the Risk Assessment Procedure.
