Payroll deductions form

With the exception of deductions allowed under relevant laws (e.g. taxes, child support etc.), all other payroll deductions must have written permission from the employee prior to a deduction being made. This Payroll deduction form may be used by an employee to permit or request payroll deductions to be made.
