This form is to be used to request an extension of unpaid parental leave for a further period of up to 12 months immediately following the end date of the initial 12-month period (up to 24 months in total). When an eligible employee makes a request for an extension of unpaid parental leave, the employer has an obligation to discuss the request with them.
